California State Preschool Program (CSPP) contractors who received written approval of CSPP Neighborhood School Eligibility from the California Department of Education (CDE) between 2019–20 and 2022–23 must complete this annual reverification survey to determine CSPP Neighborhood School Eligibility for 2023–24. The survey must be completed separately for each CSPP site seeking to reverify, and all questions are required.
The survey asks for:
- Contractor name
- Vendor number
- Site name (as it appears in the Child Development Management Information System [CDMIS])
- Qualified free or reduced-price meal (FRPM) school previously used to establish eligibility and its school district
- Information regarding any changes to the school’s address and/or boundaries
Additionally, contractors will need to identify if the school is on the 2023–24 List of Eligible Public Elementary Schools, posted to the CSPP Neighborhood School Site Eligibility web page. |
