To prepare for the implementation of Universal Meals beginning in School Year (SY) 2022–23, the Nutrition Services Division (NSD) is asking that you complete this survey. The purpose of this survey is to help the NSD identify how many school agencies are planning to apply to participate in either the Community Eligibility Provision (CEP) or Provision 2 (P2). This data will help the NSD identify additional technical assistance and support that may be needed for local program operators.
This survey should take approximately three minutes to complete. In order to submit this survey, you will need to complete all of the data fields. If you have questions or technical difficulties, please contact the NSD by email at SNPinfo@cde.ca.gov for assistance.
If your school or district is not interested in applying to participate in either CEP or P2, you can disregard this survey.
