Model Community Day School Recognition Program
(Program Year 2024–25)

This form is to be used by principals of community day schools to indicate their intent to submit a 2024–25 Model Community Day School (MCDS) Application. Submission of this completed form is required in order to be considered for model school status.

Submitting this form
does not obligate the principal to submit an application. All questions in this survey are required unless marked as optional.
School Eligibility Criteria:
  1. The school is established as a community day school according to California Education Code sections 48660–48666.
  2. It is recommended that the school be accredited by the Western Association of Schools and Colleges (WASC). Schools that are not WASC accredited are still eligible to apply.
    1. If accredited, the school’s WASC Visiting Committee Report must verify that the school has completed a full self-study, which typically involves a three- and one-half-day visit.
