The 2022–23 Fresh Fruit and Vegetable Program (FFVP) Midyear Assessment Report Survey is designed to assess program implementation, fiscal status, and technical assistance (TA) needs of the 2022–23 FFVP grantees.
The Nutrition Services Division (NSD) will use survey responses to provide targeted TA to the 2022–23 FFVP grantees during the last part of the grant period.
This survey has 17 questions and should take approximately 20 minutes to finish in a single session. In order to complete this survey, all questions must be answered.
The 2022–23 FFVP Midyear Assessment Survey is mandatory. A lack of response by the due date may jeopardize future FFVP funding.
