Post Survey: California Farm to Summer Celebration Week 2025 (June 16–20)  

Nutrition Services Division

As the last step in your Farm to Summer (F2Summer) Celebration week participation, please complete this post-survey by Friday, September 26, 2025, at 5 p.m. Your responses will help the California Department of Education (CDE) Nutrition Services Division (NSD) understand the Celebrations successes and challenges during summer 2025.

  • Each school food authority (SFA) completes one survey for all participating sites.

  • Responses included in the survey are not confidential.

  • Please avoid providing personally identifiable information in your responses.

  • Photos uploaded may be edited and/or shared by the CDE NSD, and are not confidential.

  • Photo releases are required for any submitted photos that include recognizable faces. Do not submit photos of students.

  • There is no guarantee the CDE NSD will utilize the photos uploaded.

  • All questions are required unless marked optional.

  • This survey should take 10–15 minutes to complete.

    Please Note: The Save Responses button allows you to save your place in the report and return to it later. When you select the Save Responses button, a new screen will appear, and you will be provided with two options that will allow you to access the saved report: 1) save the reports unique URL as a bookmark or add it as a “Favorites” in your web browser or 2) provide your email address and you will be emailed a unique URL.
