Alternative Schools and Programs of Choice
Annual Evaluation

Whole Child Division

The law requires that alternative schools established under California Education Code sections 58500–58512 prepare an annual evaluation report.

Therefore, all Alternative Schools and Programs of Choice are required to submit one completed report based on a compilation of the student, parent, and teacher surveys collected by the administrator.

The information provided in this report must reflect the 2024–25 school year.

The report is due on or before August 1, 2025.
All fields are required to be filled in, except "if applicable", to complete this report.
Users may save the report and resume later by clicking on the Save Responses* button. The Save Responses* offers three options for saving. 1) Copy and save the URL; 2) Bookmark the current report page to return to the report; and 3) provide an email address and a link will be sent to the email address provided.

*Note: Using the
Save Responses button does not save the responses for your records. To keep a copy of the responses for your records, users must click on the Print Responses button at the end of the report before clicking on the Submit button.
This report should take approximately 20 minutes to complete if all student, parent, and teacher surveys have been tallied before completing this report.
